The Spen Health and Wellbeing Primary Care Network is an alliance of seven practices collectively caring for patients within the Cleckheaton, Liversedge and Heckmondwike area.

The Spen Health & Wellbeing Primary Care Network aims to ensure its population has personalised access to a high quality service that is appropriate to the needs of its patients with a focus on prevention and helping patients to manage their health and wellbeing. Our PCN aims to use its resources to help support the practices to deliver high quality care, improve resilience and meet their contractual requirements.
